Sports Physicals

The Health Services Office staff plays an integral role in MSAD#1 Sports Program. All MSAD#1 students participating in school sports are cleared to participate through the Health Services Office. In the spring of the year, the Health Services Office provides the opportunity for students to have their required Sports Physical during the school day in conjunction with local health care providers.

School Sports Physical Exam (Annual Form A) to be completed by medical provider

Student Yearly Sports Health History (Annual Form B) to be completed by parent

Permission for emergency medical care (Annual Form C) to be completed by parent
